 		 Medical
  • Class 1 and 2 Medical Forms - This form is used by most Scouts and adults under the age of 40.  The health history has to be updated and signed annually.  The doctor's physical is good for three years.
  • Class 3 Medical Forms - This form is used by adults over the age of 40 and Scouts participating in the High Adventure program.
  • Prescription Medication Information Form - Scouts taking medication at camp need to have this form filled out and turn in during Medical Rechecks.
